Job Requirements

The Relief Support Worker will be responsible for providing high-quality support services to service users, working collaboratively with colleagues to achieve individualized goals and outcomes.

Key Responsibilities
  • Develop and maintain relationships with service users, their families, and other stakeholders.
  • Conduct risk assessments and develop individualized support plans, working closely with colleagues and professionals from other agencies.
  • Monitor and evaluate the effectiveness of support plans, making adjustments as needed to ensure optimal outcomes.
