Job Summary:

This is a full-time position that offers a competitive salary of £30,000 - £40,000 per annum, depending on experience. The successful candidate will be responsible for coordinating the care of patients with complex needs, working closely with healthcare professionals to ensure seamless transitions between services.

Responsibilities:

  • Coordinate patient care plans and communicate with healthcare professionals to ensure smooth transitions between services
  • Attend regular MDT meetings to discuss patient progress and identify areas for improvement
  • Analyze data to inform care decisions and identify trends in patient outcomes
  • Work closely with community teams to ensure effective care delivery and coordination

Requirements:

  • Minimum 2 years' experience working in a healthcare setting
  • Excellent communication and organizational skills
  • Experience working with healthcare databases and analyzing data
  • Ability to work effectively in a fast-paced environment
