Healthcare Support Specialist

We are seeking a skilled and compassionate Care Coordination Manager to join our team at PML. As a key member of our Integrated Neighbourhood Team, you will play a vital role in supporting the community primary care team to collaborate effectively and improve patient care.
The successful candidate will be responsible for managing the care of people registered with practices within our Primary Care Network (PCN). You will contribute to tackling inequalities in health and social care, particularly regarding individuals with long-term conditions.
You will run daily and weekly Multi-Disciplinary Team (MDT) meetings, identifying and reviewing patients recently discharged, at risk of admission, or with significant frailty or specific MDT needs. You will also provide coordination, secretarial, and administrative support to the MDTs within the INT.
