We are seeking a dedicated and compassionate Care and Support Practitioner to join our team. The ideal candidate will be committed to delivering high-quality care and have excellent interpersonal, communication, and organisational skills.
Vacancy Details:
- This is a full-time post with a salary range of £18,000 - £25,000 per annum, dependent on experience.
- The successful candidate will work collaboratively with all members of the team to provide person-centred care and support to individuals in need.
About the Role:
- Deliver high-quality care and support to individuals in need.
- Assess and identify individual needs and develop tailored care plans.
- Maintain accurate records of patient interactions and progress.
Requirements:
- Degree in Social Work or related field.
- Experience working with vulnerable adults.
