Transition Support Specialist

Pertemps Network Group is seeking a skilled Transition Support Worker to join their team in Tameside. This rewarding role offers the opportunity to make a positive impact on the lives of young people, supporting them to achieve their goals and reach their full potential.
As a Transition Support Worker, you will work closely with young people referred to the project, engaging them with the service and providing support to help them overcome challenges. Your expertise will be invaluable in developing individual plans to address their future aims and objectives, and working with them to access education, training, and employment opportunities.
Main responsibilities include:
- Working within statutory guidelines and departmental procedures relating to young people leaving care, children in need, Cared for Children, and asylum-seeking young people.
- Undertaking direct work with young people and others relevant in their lives, building supportive and empowering relationships.
- Responding appropriately to child protection/Adult at Risk concerns, following Safeguarding Policy and Procedures.
- Liaising with other agencies on behalf of young people, maintaining trust and confidence, and linking them into services and support appropriate to their needs.
- Attending forums related to planning for and supporting young people in housing, health, Further/Higher Education, Training, and Employment.
The estimated salary for this role is £16.01 per hour, reflecting the importance of your contributions to the team.
