Role Summary: We are seeking a dedicated Transition Support Worker to join our team in Tameside. The successful candidate will work closely with young people who have left care, providing them with the support and guidance they need to live independently.
">About the Role: As a Transition Support Worker, you will be responsible for building strong relationships with young people, helping them develop essential life skills, and providing guidance on accessing local services and resources.
">Key Responsibilities:
- Support young people aged 16-25
- Help them develop independence skills
- Guide them on accessing local services and resources
Requirements: A qualification in childcare to level 3, experience working with adolescents, and knowledge of supporting individuals with vulnerability and complex needs.
">Salary Information: The estimated salary for this position is approximately £16,071 per annum, based on the pay rate of £16.01 per hour.
