Job Title: Youth Transition Support Coordinator

The Talent Crowd is recruiting an experienced Youth Transition Support Coordinator to join our team. As a Youth Transition Support Coordinator, you will be responsible for providing crucial support to young people transitioning into adulthood.

**Job Overview:**
This role involves working closely with care leavers, ensuring they receive the necessary support to overcome the challenges they face. The successful candidate will have a deep understanding of the issues affecting young people in care and the ability to build trust.

**Key Responsibilities:**
- Deliver high-quality support to care leavers, addressing their specific needs and concerns
- Collaborate with local services to guarantee consistent support for young people
- Develop and maintain awareness of local job market trends and career development opportunities
- Establish and maintain effective relationships with key stakeholders, including employers and education providers

**Requirements:**
- Comprehensive understanding of the needs and challenges faced by care leavers
- Outstanding communication and interpersonal skills
- Capacity to work independently and as part of a team
- Familiarity with local job market trends and career development opportunities

**Salary:** £19.50 per hour (36 hours per week)
