Job Title: Mental Health Social Care Specialist
Location: Ashton-Under-Lyne
Salary: £35 per hour (assignment rate)
About the RoleWe are seeking a qualified and experienced Social Worker to provide high-quality social care services to adults of working age experiencing moderate to severe mental health problems within the community.
Main Responsibilities:
- To deliver comprehensive social care services to adults with moderate to severe mental health issues.
- To develop clear support plans following a Care Act assessment, complete funding applications, and perform tasks with legal requirements such as Copdol, Dols, 21a's, and reports for court.
- To support carers through the Care Act, work with children about to turn 18, and transition them into adult mental health services.
Requirements:
- Degree or equivalent in social work.
- Current Social Work England registration.
- Significant frontline experience in Adults Social Services within the UK as a qualified Social Worker.
