Join Pennine Care NHS Foundation Trust as a Senior Mental Health Practitioner, working within the Living Well Service in Tameside & Glossop. This exciting opportunity involves collaborating with a multidisciplinary team to identify and support patients experiencing mental health challenges in the community.

As a senior mental health practitioner, you will conduct comprehensive biopsychosocial assessments and initial conversations with individuals presenting with mental health difficulties, social care needs, and risk issues at a primary care level. Your primary goal will be to promote recovery and help individuals maintain their lives within their community environment.

Main Duties of the Job

The key responsibilities of this role include:

  • Conducting biopsychosocial assessments and initial conversations with individuals presenting with mental health difficulties, social care needs, and risk issues at a primary care level.
  • Developing and implementing risk management plans with service users as part of the duty function.
  • Providing brief interventions, including devising coping strategies, problem-solving, motivational and behavioral activation work, anxiety management, risk management, and suicide prevention work.
  • Offering medication support, promoting social and interagency support, providing psychological support, and biopsychosocial education aimed at insight development.
  • Reviewing treatment plans with service users deemed too complex for other parts of the Living Well Service or who may not meet the criteria for secondary care services.
Person Specification

The ideal candidate will possess:

  • RMN/RNMH/Social Worker/OT or equivalent qualifications.
  • Evidence of ongoing post-registration experience.
  • Desirable qualifications include a degree, completion of Preparation for Mentorship/Practice Teacher/Fieldwork Educator, and further training in brief psychosocial interventions.
