Job Summary

This is an exciting opportunity to join our team as a Care Manager, working in a 10-bedded female unit providing intensive rehabilitation based on a gender-sensitive approach to care. Our aim is to enhance life skills and community functioning, support individuals to effectively manage risk behaviors, reduce relapses, and maximize their independence.

Main Responsibilities
  • Provide comprehensive nursing assessments and co-produce care plans in consultation with service users and their families and carers.
  • Undertake comprehensive risk assessments and formulate risk management plans with service users, their families, and carers.
  • Assist the Unit Manager in developing a service that meets the needs of patients regarding their age, spiritual, cultural, ethnic, and sexual orientation.
  • Liaise effectively with the multi-disciplinary team and statutory and non-statutory organizations involved in client care.
Requirements

To be successful in this role, you will require:

  • A Registered Mental Health Nurse qualification.
  • Experience of working within mental health settings, including experience of admission and discharge processes.
  • Ability to negotiate constructively and communicate effectively.
  • Excellent interpersonal skills and evidence-based practice.
