About the Role

We are seeking a highly skilled and experienced mental health professional to join our Home Treatment Team as a Senior Mental Health Practitioner. The successful candidate will be responsible for providing intensive home treatment to individuals experiencing mental health crises, working closely with a multidisciplinary team to develop and implement individualized care plans.

Key Responsibilities

  • Conduct comprehensive mental health assessments and develop care plans in collaboration with service users, carers, and other healthcare professionals.
  • Provide intensive home treatment, including crisis management and short-term psychotherapeutic interventions.
  • Liaise with other healthcare professionals, statutory agencies, and independent organizations to ensure seamless care coordination.
  • Develop and maintain relationships with service users, carers, and other stakeholders to promote recovery-focused care.
  • Contribute to the development and implementation of team policies and procedures, ensuring compliance with relevant legislation and guidelines.

Requirements

  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker with extensive experience in mental health crisis intervention.
  • Strong knowledge of mental health legislation, guidance, and best practices.
  • Excellent communication, interpersonal, and leadership skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Commitment to ongoing professional development and evidence-based practice.
