Our Team:

We are a diverse and dynamic team of healthcare professionals dedicated to providing exceptional care to our patients. As a Clinical Care Coordinator, you will be an integral part of our team, working closely with colleagues to ensure seamless care delivery.

Your key responsibilities will include:
  • Coordinating patient care through effective communication and collaboration with multidisciplinary teams.
  • Developing and implementing personalized care plans to promote independence and reduce hospital stays.
  • Maintaining accurate records and reporting any issues as necessary.

Requirements:
  • Degree or equivalent qualification in Occupational Therapy.
  • Registration with the HCPC.
  • Excellent communication and interpersonal skills.
  • Experience working in a variety of settings, including community-based roles.

Salary: £40,000 - £60,000 per annum, depending on experience.
