The ideal candidate will have a strong background in occupational therapy and experience working with patients with complex needs. They will be able to take a holistic and person-centred approach to supporting patients to recover from their recent hospital admission and achieve their short-term goals, maximising independence.
The role will involve visiting patients in their homes to perform assessments and treatments, as well as providing advice, guidance, and information to health and social care professionals.
