Role Overview

We are seeking an experienced Housing Support Specialist to join our team as an Outreach Practitioner. In this role, you will work closely with homeless individuals and vulnerably housed groups to provide harm reduction and brief interventions, as well as developing and implementing care plans tailored to their unique needs.

Responsibilities

  • Deliver satellite provisions to co-locations and community settings.">
  • Collaborate with multidisciplinary teams to develop and implement effective care plans.">
  • Maintain accurate records and documentation, ensuring compliance with regulatory requirements.">
