The Job Description

We are seeking an experienced and empathetic Street Outreach Worker to join our team at Snow Hill Court. As a key member of our Rapid Response Team, you will be responsible for conducting outreach activities, assessing client needs, and developing personalized support plans. Your exceptional communication skills, ability to build rapport with clients, and experience working with vulnerable populations will be crucial in creating a positive impact.

You will:

Main Responsibilities:
  • Carry out outreach activities to engage with clients and assess their needs
  • Develop and implement personalized support plans to address client goals and objectives
  • Liaise with internal and external partners to ensure seamless service delivery
  • Maintain accurate records and reporting on client progress

We offer a competitive salary of £28,000 - £32,000 per annum, depending on experience, plus a range of benefits, including 29 days' annual leave, pension scheme, and access to ongoing training and development opportunities.
