About This Role

We are seeking a highly skilled Community Outreach Worker to join our team at Thames Reach. As a key member of our outreach team, you will play a crucial role in supporting individuals who are homeless or at risk of homelessness.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to form effective relationships with clients and partner agencies.

Our outreach teams operate across London, responding rapidly to help those who are rough sleeping find routes away from the street.

This is a challenging yet rewarding role, offering the opportunity to make a significant difference in people's lives.

Key Responsibilities:

  • Assess the support needs of new clients using a person-centred approach.
  • Manage a caseload of clients, identifying the most appropriate route towards sustainable and permanent accommodation.
  • Work alongside various partner agencies to ensure clients access suitable support for housing.

Requirements:

  • Strong communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Commitment to supporting people towards independence.
  • IT skills, including Outlook, online recording systems, Excel, and Word.
