Job Description: Outreach Support Worker

Polaris Community is seeking a highly motivated and compassionate individual to fill the role of Outreach Support Worker.

As an Outreach Support Worker, you will play a vital role in delivering personalized support services to young people, promoting their independence and well-being.

Main Responsibilities:
  • Provide daily support to young people, including attending appointments, participating in activities, and offering guidance and advice.
  • Work collaboratively with young people, families, and external agencies to develop and implement effective support plans.
  • Monitor and evaluate the effectiveness of support services, making recommendations for improvement where necessary.
  • Engage in ongoing professional development to enhance knowledge and skills in supporting vulnerable young people.
Required Qualifications and Experience:
  • Level 3 Diploma in Health and Social Care or equivalent qualification.
  • Proven experience in supporting young people in a residential or community setting.
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with young people and families.
  • Ability to work flexibly, including weekends and evenings, to meet the needs of young people.
