Job Overview
Cumbria Health is seeking a skilled Advanced Clinical Practitioner to join our Out of Hours team. In this role, you will work as a member of a multi-disciplinary team, providing high-quality care to patients in our treatment centers.
Key Responsibilities
- Work independently as an Advanced Clinical Practitioner, using your skills and experience to assess and manage patients with undifferentiated presentations.
- Communicate effectively with patients, carers, and other healthcare professionals to deliver patient-centered care.
- Contribute to the development and delivery of a high-quality, innovative service to the community, using evidence-based clinical practice and continuous professional development.
- Participate in clinical governance, clinical audit, and clinical risk assessment, setting and monitoring standards of care.
- Work collaboratively with other healthcare professionals to ensure safe and effective patient care.
- Deliver a high standard of care without discrimination to all ages, sexes, and ethnic origins of the population.
- Promote improving the health of the individual through advice and education to aid adjustment of lifestyle.
