About the Role

We are seeking a highly skilled and experienced professional to join our team as a Continuing Healthcare Specialist. As part of NHS Dorset Personal Health Commissioning Team, you will play a key role in supporting the delivery of high-quality care to individuals who require Continuing Healthcare.

Key Responsibilities

  • Manage a caseload of CHC eligible individuals in accordance with the National Framework for Continuing Healthcare
  • Undertake eligibility reviews and assess individuals' needs to identify the most appropriate care options
  • Assist NHS Dorset commissioners in searching for care and make recommendations on suitable placements

Requirements

  • Extensive knowledge of specialist areas acquired through experience and training, plus further specialist knowledge of the Continuing Healthcare process
  • Good understanding of Personal Health Commissioning
  • Experience of working in a health environment, with negotiation and conflict management skills and the ability to influence
  • Previous experience of caseload management

About Us

NHS Dorset operates a Monday-Friday 9-5 service and currently offers a hybrid model of working with a percentage of contracted hours being on-site and the remainder being home-based. We are part of the Dorset Integrated Care System (ICS), which brings together health and care services to improve population health and reduce health inequalities.
