About the Role

We are seeking an experienced and skilled Senior Practitioner to join our team at NHS Dorset. As a Senior Practitioner, you will play a key role in supporting the Personal Health Commissioning Service, working within the remit of the National Frameworks for Continuing Healthcare.

Key Responsibilities
  • Manage a caseload of CHC Eligible individuals, ensuring The National Framework for NHS Continuing Healthcare and Funded-Nursing Care is implemented and complied with, according to the principles and processes.
  • Oversee a caseload of complex cases, undertake annual eligibility reviews and manage Continuing Healthcare packages of care, ensuring value for money placements which meet the needs of the individual and identify opportunities for development and improvement to assist delivery of best practice healthcare.
  • Assist NHS Dorset commissioners in searching for care and make recommendations on most appropriate placement.
Requirements
  • Extensive knowledge of specialist areas acquired through experience and training, plus further specialist knowledge of the Continuing Healthcare process.
  • Good understanding of Personal Health Commissioning.
  • Experience of working in a health environment.
  • Negotiation and conflict management skills and the ability to influence.
  • Previous experience of caseload management.
Skills and Abilities
  • Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making.
  • Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales.
  • Ability to analyse very complex cases where material is conflicting and drawn from multiple sources.
  • Effective communication skills.
  • Evidence of continued professional development.
  • Ability to deliver professional standards in all recording and assessment reports.
Qualifications
  • Educated to degree level in a relevant subject or equivalent level of experience working at a similar level in a specialist area, ideally Personal Health Commissioning.
  • Nursing qualification and registered with a professional body.
  • Knowledge of NHS Personal Health Commissioning and associated procedures and practices, underpinned by theoretical knowledge of the National Framework for NHS Personal Health Commissioning and NHS-funded Nursing Care and Childrens and Young Peoples Continuing Care.
  • Evidence of Continued Professional Development.
