As part of NHS Dorset Personal Health Commissioning Team (PHC),you will manage a caseload of CHC eligible individuals in accordance withNational Framework for Continuing Healthcare, Undertake eligibility reviews andassess individuals needs in order to identify most appropriate care options.You will assist NHS Dorset commissioners in searching for care and makerecommendations on appropriate placements.

Main duties of the job

You will manage a caseload of CHC eligible individuals inaccordance with National Framework for Continuing Healthcare, collaboratingwith a range of healthcare professionally, community and acute services.

You must be able to work flexibly and autonomously and workcollaboratively with Health and Social Care practitioners, patients, families,and carers, ensuring high quality assessments and timely service is delivered.You must demonstrate an openness and honesty and have strong assessment andcommunication skills and a knowledge of the National Frameworks for ContinuingHealthcare, the Mental Capacity Act and Mental Health Act and Care Act2014.

PHC has 2 offices. East Team is based in Poole, coveringBournemouth, Christchurch, Poole area, and the West Team is based inDorchester, covering Dorset area. This job advert is recruiting to positionswithin the East locality and based in Wallisdown, Poole, however you may berequired to provide support across both sites as necessary. There will also bea requirement to make off-site visits to undertake assessments and attendmeetings and it is therefore essential that you have a full driving licence withaccess to a vehicle for business use.

NHS Dorset operates a Monday-Friday 9-5 service.

The post holder will play a key role insupporting the Personal Health Commissioning Service. Working within the remitof the National Frameworks for Continuing Healthcare, the post holder willdeliver an effective and competent level of service and consistently deliver aclient-focused service.

The post holder will be required to managea caseload of CHC Eligible individuals ensuring The National Framework for NHS ContinuingHealthcare and Funded-Nursing Care is implemented and complied with, accordingto the principles and processes.

The post holder will oversee a caseload of complex cases, undertake annualeligibility reviews and manage Continuing Healthcare packages of care, ensuringvalue for money placements which meet the needs of the individual and identifyopportunities for development and improvement to assist delivery of bestpractice healthcare.

The post holder willassist NHS Dorset commissioners in searching for care and make recommendationson most appropriate placement.

This is not a patient facing role, and ispredominantly desk based, requiring high levels of concentration and an abilityto analyse health, social care, and other agency records to identify whetherthe individual has/had a Primary Health Care Need, in accordance with theNational Framework for Continuing Health Care.

NHS Dorset currently offers a hybrid modelof working with a percentage of contracted hours being on site and theremainder being home based.

Extensive knowledge of specialist areas acquired through experience and training plus further specialist knowledge of the Continuing Healthcare process Good understanding of Personal Health Commissioning Experience of working in a health environment Negotiation and conflict management skills and the ability to influence Previous experience of caseload management

Skills, abilities and knowledge

Essential

Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making. Demonstrated capabilities to manage own workload effectively and make informed decisions in the absence of required information, working to tight and often changing timescales Ability to analyse very complex cases where material is conflicting and drawn from multiple sources Effective communication skills Evidence of continued professional development Ability to deliver professional standards in all recording and assessment reports

Qualifications

Essential

Educated to degree level in relevant subject or equivalent level of experience working at a similar level in specialist area, ideally Personal Health Commissioning Nursing qualification and registered with a professional body. Knowledge of NHS Personal Health Commissioning and associated procedures and practices, underpinned by theoretical knowledge of the National Framework for NHS Personal Health Commissioning and NHS-funded Nursing Care and Childrens and Young Peoples Continuing Care Evidence of Continued Professional Development
