Job Role Summary
This is an exciting opportunity to join the Personal Health Commissioning Team at NHS Dorset as a Continuing Healthcare Specialist. The successful candidate will manage a caseload of CHC eligible individuals, working in accordance with the National Framework for Continuing Healthcare.
Main Responsibilities
- Manage a caseload of CHC eligible individuals, undertaking eligibility reviews and assessments to identify the most appropriate care options.
- Assist NHS Dorset commissioners in searching for care and making recommendations on suitable placements.
- Work collaboratively with healthcare professionals, community, and acute services to deliver high-quality assessments and timely services.
About the Role
This is a challenging yet rewarding role that requires the ability to work autonomously and flexibly, with strong assessment and communication skills. The successful candidate will have a good understanding of Personal Health Commissioning and the National Framework for Continuing Healthcare.

Person Specification
- Extensive knowledge of specialist areas acquired through experience and training.
