About the Role
NHS Dorset is seeking a highly skilled and experienced healthcare professional to join our Personal Health Commissioning Team as a Continuing Healthcare Specialist. As a key member of the team, you will play a vital role in supporting the commissioning of high-quality, person-centred care for individuals with complex needs.
Key Responsibilities
- Manage a caseload of Continuing Healthcare eligible individuals, ensuring timely and effective assessments and eligibility reviews.
- Collaborate with healthcare professionals, community services, and acute services to deliver high-quality care and support.
- Develop and maintain strong relationships with individuals, families, and carers to ensure their needs are met and their voices are heard.
- Work closely with NHS Dorset commissioners to identify and secure the most appropriate care options for individuals.
Requirements
- Extensive knowledge of Continuing Healthcare processes and procedures.
- Experience of working in a health environment, with a strong understanding of Personal Health Commissioning.
- Excellent communication and negotiation skills, with the ability to influence and manage complex casework.
- Ability to work autonomously and as part of a team, with a strong focus on delivering high-quality care and support.
