About the Role

We are seeking an experienced and skilled Senior Practitioner to join our Personal Health Commissioning Team at NHS Dorset. As a key member of our team, you will play a vital role in supporting the commissioning of high-quality healthcare services for individuals with complex needs.

Key Responsibilities
  • Manage a caseload of Continuing Healthcare eligible individuals, ensuring that their needs are assessed and met in accordance with the National Framework for Continuing Healthcare.
  • Collaborate with healthcare professionals, community services, and acute services to deliver high-quality assessments and timely services.
  • Develop and maintain effective relationships with stakeholders, including patients, families, carers, and healthcare providers.
  • Contribute to the development and implementation of commissioning strategies to improve healthcare outcomes and reduce health inequalities.
  • Work closely with NHS Dorset commissioners to identify and secure appropriate care placements for individuals with complex needs.
Requirements
  • Extensive knowledge of Continuing Healthcare and the National Framework for Continuing Healthcare.
  • Experience of working in a health environment, with a strong understanding of commissioning and healthcare services.
  • Excellent communication and negotiation skills, with the ability to influence and manage conflict.
  • Ability to analyze complex cases and make informed decisions in the absence of complete information.
  • Strong analytical and problem-solving skills, with the ability to work effectively in a fast-paced environment.
