About the Role

NHS Dorset is seeking an experienced and skilled Senior Practitioner to join our Personal Health Commissioning Team. As a Senior Practitioner, you will play a key role in supporting the commissioning of Continuing Healthcare services, working closely with healthcare professionals, community and acute services to ensure high-quality assessments and timely service delivery.

Key Responsibilities
  • Manage a caseload of CHC eligible individuals, ensuring the National Framework for NHS Continuing Healthcare and Funded-Nursing Care is implemented and complied with.
  • Oversee complex cases, undertake annual eligibility reviews, and manage Continuing Healthcare packages of care, ensuring value for money placements that meet the needs of the individual.
  • Assist NHS Dorset commissioners in searching for care and make recommendations on most appropriate placements.
Requirements
  • Extensive knowledge of specialist areas acquired through experience and training, plus further specialist knowledge of the Continuing Healthcare process.
  • Good understanding of Personal Health Commissioning.
  • Experience of working in a health environment, with negotiation and conflict management skills and the ability to influence.
  • Previous experience of caseload management.
