Job Summary
We are looking for a skilled and compassionate Social Worker to work with adults in Cornwall, providing expert support and guidance to help them achieve their goals and improve their overall well-being. As a key member of our team, you will be responsible for conducting assessments, developing support plans, and collaborating with other professionals to ensure the best possible outcomes for our clients.

Key Responsibilities
* Conduct thorough assessments of adult service users, identifying their needs and developing tailored support plans
* Work collaboratively with other professionals, including healthcare teams and family members, to deliver effective support and care
* Develop and maintain strong relationships with service users, their families, and carers
* Utilize Microsoft Teams and Mosaic to facilitate communication and record-keeping
* Contribute to the development and implementation of new initiatives and projects that promote the well-being of adults in Cornwall

Requirements
* Registered Social Worker with experience working with adult service users, including those with learning disabilities and older adults
* Proven ability to conduct assessments, develop support plans, and work collaboratively with others
* Strong communication and interpersonal skills, with the ability to build rapport with diverse groups
* Proficient in using Microsoft Teams and Mosaic software
