Community Mental Health Specialist

We are seeking a qualified Social Worker to join our Community Mental Health Team in Caerphilly. As a key member of our team, you will work closely with individuals recovering from serious mental health issues, providing assessment, review, and care coordination services.
Key Responsibilities:- Act as assessor, reviewer, and care coordinator for a caseload of individuals with mental health needs.
- Participate in the duty desk rota and multi-disciplinary team meetings, assessment clinics, and ward rounds as required.
- A minimum of 6 months' experience of working in the UK as a registered Social Worker.
- A relevant Social Work Qualification.
- Registration with Social Care Wales or Social Work England.
- Enhanced DBS, registered on the update service, or willing to apply for a DBS.
- 3 years' worth of referencing.
- Driving license with access to a car.
- Good flexible working policy.
