Job Title: Community Mental Health Team Social Worker

We are seeking a qualified Social Worker to join our Community Mental Health Team in Caerphilly. As a key member of our team, you will work closely with individuals recovering from serious mental health issues, providing assessment, review, and care coordination services.

Key Responsibilities:
  • Act as assessor, reviewer, and care coordinator for a caseload of individuals with mental health needs.
  • Participate in the duty desk rota and multi-disciplinary team meetings, assessment clinics, and ward rounds as required.
Requirements:
  • A minimum of 6 months' experience of working in the UK as a registered Social Worker.
  • A relevant Social Work Qualification.
  • Registration with Social Care Wales or Social Work England.
  • Enhanced DBS, registered on the update service, or willing to apply for a DBS.
  • 3 years' worth of referencing.
  • Driving license with access to a car.
