About the Role

We are seeking a highly motivated and empathetic Care Experienced Peer Mentor to join our team at Polaris Community. As a Care Experienced Peer Mentor, you will play a vital role in supporting children and young people who are transitioning into foster care or reunifying with their families.

Key Responsibilities
  • Support Transition Periods: Provide guidance and support to children and young people before, during, and after their transition period to foster care or reunifying with their families.
  • Act as a Positive Role Model: Share your own care experiences and use them as a positive influence to help children and young people navigate their own challenges.
  • Advocate for Children's Wishes: Work with a professional network, including foster carers, social workers, and schools, to ensure the child's or young person's wishes and feelings are heard and respected.
  • Community Visits: Conduct regular face-to-face and telephone contact with children and young people, including visits to their homes and community settings.
  • Equality and Diversity: Ensure that the service delivered promotes equality, diversity, and rights for all individuals associated with the service.
Requirements
  • Full Driving Licence: Possess a full driving licence with unrestricted access to your own car.
  • Flexibility: Be flexible and able to work in a fast-paced environment.
  • DBS Check: Complete an enhanced DBS check, which will be met by Polaris Community.
