About the Role

We are seeking a skilled Mental Health Support Worker to join our Home Treatment Team at Devon Partnership NHS Trust. The successful candidate will work in a community-based setting, providing support to individuals experiencing severe mental health issues.

Key Responsibilities

  • Carry out Home Treatment Team visits and telephone calls, working collaboratively with service users, families, and carers.
  • Develop and implement interventions from collaborative care plans, including signposting to appropriate agencies.
  • Provide therapeutic engagement to service users in crisis, supporting them to move away from crisis point and focus on desired future goals.

Requirements

  • Experience of working within a secondary mental health care setting.
  • Experience of working with individuals in acute mental health crisis.
  • Excellent communication skills, with the ability to communicate effectively with people using services.
