Job Overview

We are seeking a skilled and compassionate Mental Health Crisis Support Specialist to join our Home Treatment Team based at North Devon District Hospital. As a key member of our team, you will work closely with individuals experiencing severe mental health issues, providing therapeutic support and interventions to help them manage and resolve their crisis.

You will be responsible for carrying out home visits and telephone calls, working collaboratively with families and carers to develop and implement effective care plans. Your excellent communication skills and ability to work autonomously will enable you to provide high-quality support to our service users, while also contributing to a positive and inclusive team environment.

Key Responsibilities
  • Carry out home visits and telephone calls to provide therapeutic support and interventions to individuals experiencing mental health crisis
  • Work collaboratively with families and carers to develop and implement effective care plans
  • Provide signposting to appropriate agencies and create future support plans, safety plans and complete physical health observations as required
  • Participate in multi-disciplinary team meetings and contribute to the development of care plans
  • Work within the bounds of existing DPT policies and procedures
Person Specification

We are looking for an individual with experience of working within a secondary mental health care setting, particularly in acute mental health crisis situations. You will have excellent communication skills, with the ability to communicate effectively with people using services who may have communication difficulties. You will also have knowledge of safeguarding policy and procedures.

A UK driving license is required to carry out home visits in pool cars. We offer a competitive salary of £28,000-£34,000 per annum, depending on experience, plus a range of benefits including clinical supervision, training and continued professional development opportunities.
