Job Summary
We are seeking a highly organized and detail-oriented individual to join our Somerset Hub for Coordinating Care team as a Patient Transfer Facilitator. This exciting opportunity will allow you to work closely with clinicians to facilitate a smooth referral process for service users and patients.
Main Duties
As a Patient Transfer Facilitator, you will be responsible for:
- Working closely with clinicians to facilitate a smooth referral process for service users and patients
- Providing a single point of contact for information on patients to be transferred/admitted to community hospital beds within the Primary Care Trust
- Supporting clinicians in providing an efficient referral process, including booking transport and liaising with patients
- Working across multiple computer systems, coordinating telephone calls, and completing a range of reports

We are looking for an individual with:
- Excellent written and verbal communication skills
- Experience in the use of IT systems
- ECDL or ability to use Microsoft Word, Excel, and Outlook
- NVQ 3 or relevant experience (desirable)
