Job Summary:
We are looking for a dedicated leader to take on the role of Community Nurse. The successful candidate will be responsible for overseeing the community nursing team, ensuring seamless care delivery and high-quality patient outcomes.

What We Offer:
A competitive salary of £45,000 - £55,000 per annum, depending on experience, plus benefits.
Key Responsibilities:
- Lead the community nursing team, providing guidance and support as needed.
- Develop and implement effective care plans, ensuring patient-centered approaches.
- Maintain accurate records and reports, adhering to trust policies and procedures.
- Participate in continuous professional development, staying up-to-date with the latest clinical practices and research.
Requirements:
- Registered health care professional (NMC registration).
- Health related degree and evidence of relevant post graduate development at Level 6/7.
- Evidence of leadership experience, including team management and quality improvement initiatives.
- Valid driving licence in the UK with access to a vehicle.
