We are looking for a skilled Low Secure Hospital Social Worker to join our team at Appoint Healthcare.

About the Role:

Key Responsibilities:
  1. Develop and implement effective treatment plans.
  2. Work closely with multi-disciplinary teams to ensure seamless care delivery.
  3. Report to the Safeguarding Lead and participate in internal investigations.
  4. Liaise with home authorities during the admission and discharge process.
