Job Description

Job Title: Team Leader Support Worker

Job Summary: We are seeking a highly skilled and dedicated Team Leader Support Worker to join our team at Home not Hospital. As a Team Leader Support Worker, you will be responsible for providing hands-on care and coordinating a care team to support individuals with varying needs, including Learning Disabilities, Autism, and Mental Health needs.

Key Responsibilities:
  • Provide hands-on care and support to individuals with Learning Disabilities, Autism, and Mental Health needs.
  • Coordinate and manage the care team to ensure high-quality care and support.
  • Supervise and train care staff to ensure they have the necessary skills and knowledge to provide excellent care.
  • Conduct risk assessments and develop care plans to ensure the safety and well-being of individuals.
  • Manage rotas and ensure that care staff are adequately trained and equipped to provide care.
  • Collaborate with multidisciplinary teams to ensure that individuals receive comprehensive care and support.
Requirements:
  • Experience working with individuals with Learning Disabilities, Autism, and Mental Health needs.
  • Proven experience as a Team Leader or Support Worker in a Supported Living Setting or Residential Home.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Computer literate and accurate record keeping.
  • Driving license (must).
