Ackerman Pierce Ltd is recruiting a Youth Transition Support Worker to join our team in Devon. This is an exciting opportunity to make a difference in the lives of young people transitioning from care to independence.
About the Job:
As a Youth Transition Support Worker, you will be responsible for providing specialist support to young people leaving care, aged 16 plus. Your primary focus will be on developing close relationships with the young people, reviewing and supporting the review of Pathway Plans, and collaborating with statutory and voluntary agencies to provide a coordinated response to their needs.
Key Responsibilities:
- Working directly with care leavers to facilitate their transition to independence and adulthood
- Supporting the review of Pathway Plans to ensure the identified objectives are met
- Developing effective working relationships with key agencies and partners
- Maintaining accurate written records and participating in a duty system
Requirements and Qualifications:
- Qualified Social Worker with a SWE Registration and a degree-level education
- Excellent knowledge of therapeutic approaches, relevant legislation, national policy, and initiatives
- Experience working with Children and Families
- Eligibility to work in the UK and previous experience within Adult Social Care Teams
