We are seeking an experienced and dedicated individual to join our team as a Youth Transition Specialist. Located in Devon, this role will play a critical part in supporting young people aged 16-25 who are living in care or leaving care.

About the Role

This is a challenging yet rewarding opportunity for someone with a passion for working with vulnerable young people. As a Youth Transition Specialist, you will be responsible for providing high-quality social care support to promote the life chances of our service users.

Key Responsibilities
  • Manage and monitor a caseload of young people, prioritizing tasks and ensuring statutory responsibilities are met within timescales.
  • Develop trusting relationships with young people, understanding their needs and creating personalized plans to support their transition to adulthood.
  • Provide effective support to young people, addressing their safety, accommodation needs, education, employment, finance, and independence skills.
Requirements

To be successful in this role, you will need:

  • A sound understanding of child and adolescent development, particularly in relation to transitioning to adulthood.
  • Knowledge of relevant legislation, including the Children Act (1989), the Children Leaving Care Act (2000), and the Children and Social Work Act (2017).
  • Experience of direct work with young people, families, and carers, identifying needs and implementing service plans.
