Job Overview

We are seeking an experienced professional to join our team at Kent and Medway NHS & Social Care Partnership Trust. As a Senior Mental Health Decision-Maker, you will be responsible for ensuring the most appropriate onward pathways are identified and prompt assessment and interventions offered to patients referred to our Mental Health Together Service.

Main Duties of the Job

  1. The role involves acting as a senior decision-maker for patients referred to the Mental Health Together Service, ensuring the most appropriate onward pathways are identified and prompt assessment and interventions offered.
  2. The post-holder will act in a supervisory capacity to support practitioners in decision-making for the most complex and / or high-risk referrals.
  3. The post-holder will offer expert advice and support to all professionals referring into the Mental Health Together service and have excellent relationships with all internal and external partner agencies, including primary care, adult social care and the third sector.
  4. The post holder will work in collaboration with the Mental Health Together Team Manager to ensure the highest standards of practice and service delivery.
  5. The post holder will contribute to the transformation of community mental health services in line with the Trust's strategic objectives.
  6. The post-holder will support decision-making at all stages of a person's journey through Mental Health Together and deputise for the Team Manager in their absence.
