Personal Care Coordinator

Salary: £12.00 - £13.70 per hour
About the RoleThis permanent 36-hour contract offers an excellent opportunity to develop your skills and expertise in a supportive and dynamic environment. As a Personal Care Coordinator, you will be responsible for leading a team of Care Assistants and driving high-quality care delivery. You will work closely with the Nurse to aid with clinical duties, including observing, issuing medication, reviewing care plans, and maintaining accurate records.
Key responsibilities include:
- Leading and managing a team of Care Assistants, ensuring effective shift handovers and seamless care delivery
- Supporting with clinical duties, including assisting with medication administration and care plan reviews
- Developing and implementing individualised care plans, promoting independence and dignity for residents
- Maintaining accurate records and contributing to quality improvement initiatives
