About the Role

The post holder will play a key role across the CHC service, working within the remit of the National Framework for Continuing Healthcare, to deliver an effective, efficient and equitable service to the people and populations of Sussex.

Key Responsibilities
  • Support the CHC Team by providing a coordinated approach to the operational functions of the Front Door.
  • Manage new referrals received via the CHC portal, liaising with the wider communities and other professionals as required.
  • Progress referrals through the CHC assessment process.
  • Assist with Fast-Track referrals, case management, FNC reviews and applying the National Framework for Continuing Healthcare across Sussex.
Person Specification
  • A Professional qualification or degree in a relevant discipline or equivalent relevant work experience.
  • Good GSCE/O level passes in English and Maths, or equivalent.
  • Typing and ECDL or similar computer qualifications.
