Clinical Nurse Specialist

We are seeking a skilled and dedicated Clinical Nurse Specialist to join our team in providing high-quality care to patients in the community.
The successful candidate will be responsible for assessing, planning, and delivering clinical care to patients requiring nursing support, working closely with a multi-disciplinary team to ensure seamless care coordination.
Main Responsibilities- Conduct thorough assessments of patients to identify their care needs and develop personalized care plans.
- Work collaboratively with healthcare professionals to ensure timely and effective care delivery.
- Provide clinical expertise and support to patients and their families, promoting independence and self-management.
- Participate in quality improvement initiatives to enhance patient outcomes and service delivery.
- Develop and maintain strong relationships with community partners to ensure effective care coordination.
- Registered Nurse with a valid nursing license.
- Experience in community nursing or a related field.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work a seven-day rota, including evenings and weekends.
