The Creative Support organization is seeking a highly skilled and compassionate Care Coordinator Lead to lead our new Chorley ABI Service. This innovative service will provide tailored support for an individual transitioning from a hospital setting, focusing on empowerment, independence, and community integration. The successful candidate will play a pivotal role in ensuring excellent delivery and quality of life for the tenant.

The Care Coordinator Lead position requires a person-centered approach, strong leadership skills, and the ability to work closely with the individual, their family, and our team. Key responsibilities include service planning, team management, and collaborative working. We offer a competitive salary of approximately £38,000 - £42,000 per annum, depending on experience, along with benefits such as probationary bonus, pension contributions, 28 days leave, and company-paid enhanced DBS checks.
