We are seeking a Person-Centred Care Coordinator to join our team. This role involves providing adapted supported living services to an individual with complex needs, focusing on promoting their independence and autonomy.

The successful candidate will be responsible for:

  • Coordinating care planning and delivery
  • Developing and maintaining effective relationships with the individual, family, and carers
  • Participating in service evaluation and quality improvement initiatives

The estimated salary for this role is £20,000 - £26,000 per annum, based on location and experience.
