We are seeking a motivated and experienced individual to join our team as a Care Coordinator in Chorley, Lancashire.

About the Role

The successful candidate will work at our supported living service for adults with learning disabilities, physical needs, and support needs. Their role will be to provide consistent, positive support to enable individuals to achieve independence and outcome-focused goals within their own person-centered plan.

Main Responsibilities
  • Ensuring that all service users receive high-quality, individualized, person-centered support.
  • Providing mentorship, support, and guidance for the staff team working within the service.
  • Developing warm and trusting relationships with service users and encouraging them to express their needs, views, and concerns.
  • Supporting the management of service delivery, effectively meeting the needs of the service users.
Requirements

You will be required to work in a flexible manner, which may include working evenings and weekends.

Estimated Salary: £25,000 - £30,000 per annum
