Key Responsibilities

Your key responsibilities will include providing reactive, focused, and flexible support to adults following a hospital stay, enabling early discharge and preventing readmission. You will also respond to referrals from clinicians and community health professionals, assess service users' needs, and complete and follow a support plan for the individual.

Requirements

">

To succeed in this role, you will require a full UK manual driving licence, good knowledge of services provided by the NHS and Social Care, and the ability to make things great, improving service quality for the benefit of users.

You will receive a competitive salary of £21,840 per annum pro-rata, plus a range of benefits including 36 days annual leave, up to 6% contributory pension, and access to mental health and wellbeing assistance.
