Job Description

About the Role

We are seeking a highly motivated and compassionate Caseworker to join our team at Concept Resources. As a Caseworker, you will play a vital role in supporting individuals struggling with substance use and related issues.

Key Responsibilities

  • Manage a caseload of clients with issues around alcohol and substance use, delivering a range of psychosocial interventions to help clients achieve their health and wellbeing goals.
  • Develop and implement effective care plans, taking into account the unique needs and circumstances of each client.
  • Build strong relationships with clients, their families, and other stakeholders to ensure comprehensive support and guidance.
  • Collaborate with internal and external partners to provide a seamless service and ensure clients receive the best possible support.
  • Contribute to the development and implementation of service policies and procedures, ensuring they are aligned with best practice and regulatory requirements.
