Concept Resources is seeking a dedicated and compassionate Caseworker to join our team. As a Caseworker, you will play a vital role in supporting individuals experiencing problems with substance use, domestic abuse, and homelessness.
Key Responsibilities
- Manage a caseload of clients with issues around alcohol and substance use, delivering a range of psychosocial interventions to help clients reach their health and wellbeing goals.
- Develop and implement comprehensive care plans to address the complex needs of clients.
- Build and maintain effective relationships with clients, colleagues, and external agencies to ensure seamless service delivery.
- Conduct thorough assessments and risk management to identify and mitigate potential risks.
- Collaborate with multidisciplinary teams to provide holistic support and services to clients.
