Role Overview

Concept Resources is seeking a highly motivated and compassionate Caseworker to join our team. As a Caseworker, you will play a vital role in supporting individuals struggling with substance use and related issues.

Key Responsibilities

  • Manage a caseload of clients with substance use issues, providing a range of psychosocial interventions to help them achieve their health and wellbeing goals.
  • Deliver evidence-based programs and services to support clients in overcoming substance use challenges.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support for clients.
  • Develop and implement individualized care plans, taking into account clients' unique needs and circumstances.
  • Provide education and support to clients and their families on substance use issues and related topics.
  • Conduct regular assessments and evaluations to monitor clients' progress and adjust care plans as needed.
