Job Description

Role Overview

Concept Resources is seeking a highly motivated and compassionate Caseworker to join our team. As a Caseworker, you will play a vital role in supporting individuals struggling with substance use and related issues.

Key Responsibilities

  • Manage a caseload of clients with substance use issues, providing a range of psychosocial interventions to help them achieve their health and wellbeing goals.
  • Deliver evidence-based treatment programs, including individual and group sessions, to support clients in their recovery journey.
  • Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and other support services, to ensure comprehensive care for clients.
  • Conduct thorough assessments, develop and implement care plans, and monitor client progress to ensure effective treatment outcomes.
  • Provide education and support to clients and their families on substance use, harm reduction, and recovery strategies.
  • Develop and maintain relationships with community partners, service providers, and other stakeholders to ensure seamless referrals and support services.

  • Excellent communication and interpersonal skills, with the ability to build rapport with clients, colleagues, and community partners.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple cases effectively.
  • Resilience and adaptability in a fast-paced, dynamic environment, with the ability to manage stress and maintain a high level of professionalism.
  • A passion for working with vulnerable populations, with a commitment to social justice and human rights.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
