Role Overview
Concept Resources is seeking a highly motivated and compassionate Caseworker to join our team. As a Caseworker, you will play a vital role in supporting individuals struggling with substance use and related issues.
Key Responsibilities
- Manage a caseload of clients with substance use issues, providing a range of psychosocial interventions to help them achieve their health and wellbeing goals.
- Deliver evidence-based treatment programs, including individual and group sessions, to support clients in their recovery journey.
- Collaborate with multidisciplinary teams, including healthcare professionals, social workers, and other support services, to ensure comprehensive care for clients.
- Conduct thorough assessments, develop and implement care plans, and monitor client progress to ensure effective treatment outcomes.
- Provide education and support to clients and their families on substance use, harm reduction, and recovery strategies.
- Develop and maintain relationships with community partners, service providers, and other stakeholders to ensure seamless referrals and support services.
