Join us in our mission to support adults in living independently within their community. As a dedicated Adult Social Worker, you will form part of our Integrated Care Community Team in Lancaster and surrounding areas.

Responsibilities:
  1. Hospital Discharge: Facilitating seamless transitions from hospital to home or community by coordinating necessary support.
  2. Triage/Duty Work: Swiftly responding to urgent cases, assessing needs, and allocating resources to deliver high-quality care.
  3. Continuing Healthcare (CHC): Participating in multi-disciplinary teams to evaluate eligibility for Continuing Healthcare funding and create personalized care plans.
  4. Discharge to Assess (D2A): Conducting thorough reviews to assess ongoing care needs, guaranteeing suitable support.
  5. Interprofessional Collaboration: Collaborating with healthcare experts, caregivers, and families to foster comprehensive care coordination.

We seek applicants possessing:

  • A degree in Social Work and registration with Social Work England.
  • An enhanced Disclosure and Barring Service (DBS) check.
  • Proven experience working with adults, especially in hospital discharge, triage, or Continuing Healthcare procedures.
  • The capacity to balance office tasks with community visits.

This role offers a competitive annual salary of £32,000-£42,000.
